Donate with Checking or Savings Account
For your convenience, you can have your donation automatically transferred from your personal checking or savings account to the SMARTS account. This can be done for a one-time donation, as well as recurring monthly donations.

- Account number
- Digital signature
By signing this form, you authorize SMARTS to debit your bank account, and, if necessary, initiate adjustments for any transactions credited/debited in error.
Recurring monthly donations are processed in the middle of each month. To ensure your donation begins this month, please submit your authorization at the beginning of the month.You can opt out of your monthly donation at any time by contacting our office or sending us a written request. It’s that easy.
